Long-term Care Verifications

When applying for long-term medical assistance at your county's social services office, the following list will be provided for you.

Necessary Preparations

  1. Social Security cards for both spouses.
  2. Proof of all money received by both spouses (wages, Social Security, SSI, Veterans, etc.)
  3. Medicare Cards
  4. Blue Cross/Blue Shield or other health insurances.
  5. Last 60 months bank statements for both open and closed accounts held during that time.
  6. Verify any resources disposed of within last 60 months.
  7. All life insurance policies.
  8. Current assessment and Deeds on all property.
  9. Vehicle Registrations.
  10. Any unpaid medical bills for past 3 months.
  11. Verification of all assets (stocks, bonds, CDs, savings, etc.)
  12. Trust information verified for past 5 years.

Spouses in the community please include the following verifications for spousal allowance adjustments.

  1. Rent
  2. Mortgage
  3. Property Taxes
  4. Homeowners insurance
  5. Heat (if not included in rent)
  6. Water
  7. Gas
  8. All income received in household, S. Security, VA, pension, earned income, etc.
